To seriously be considered for this award a nominee should have a minimum average of 80% over three years of high school.

Please indicate specifically the time commitment made by the student for each point.

(Example: one weekend, once a week for 3 months, once a month for the last year. Include things such as team trainer, planning committees, grad committee, band, drama productions, open house tour guide etc…) and the main adult supervisor contact information. Please list all of the school athletic teams and involvement you have been in over their three years of high school (can also include those teams you plan to join this year).

Use chart below to state examples of UNPAID volunteer community involvement beyond the school level within the last 2 or 3 years. This may include participation in clubs or community athletic involvement.

Please write up (anywhere from a half page to a full page) as to why you deserve this scholarship (i.e. family responsibility, achievements, future education plans, sportsmanship, teamwork, leadership, etc.)

This should be a typed letter (one page maximum) from the head coach, highlighting the nominee’s assets.
