Part A - Sport Ability (Weight 40%)

List the nominee's involvement and awards/achievements.

Include awards and results of the nominee's participation pertaining to the sport being recognized OR for all sports if applying for the 3rd Year Awards including ALL SCHOOL & CLUB SPORT INVOLVEMENT.

(Examples to include are tournaments, awards, recognitions, position played, captain etc…)

To seriously be considered for this award a nominee should have a minimum average of 80% over three years of high school.

Part C - School Involvement (Weight 25%)

Use chart below to list examples of Citizenship, Sportsmanship and Leadership.

Please indicate specifically the time commitment made by the student for each point.

(Example: one weekend, once a week for 3 months, once a month for the last year. Include things such as team trainer, planning committees, grad committee, band, drama productions, open house tour guide etc…) and the main adult supervisor contact information. Please list all of the school athletic teams and involvement you have been in over their three years of high school (can also include those teams you plan to join this year).
